At Community Action Network (CAN), collaboration is at the heart of everything we do. By working together, we\u2019re tackling health inequalities and creating a more inclusive, supportive environment for all. Here\u2019s a look at some of the exciting projects making a real difference in our communities.<\/p>\n

\u2018I Am More Than\u2026\u2019 Project<\/strong><\/p>\n

We\u2019re proud to be part of the \u2018I Am More Than\u2026\u2019 partnership, now entering its third phase. Funded by NHS England through the Research Engagement Network (REN), this initiative brings together Bournemouth University, The Lantern Trust, The Healthbus, and NHS Dorset to empower charities and community groups in research.<\/p>\n

By challenging stereotypes and amplifying lived experiences, we\u2019re fostering a more inclusive society\u2014one where individuals shape research to make it more relevant and impactful. This project is all about giving people a platform to share their stories and take ownership of their health and wellbeing.<\/p>\n

Together, we\u2019re paving the way for inclusive, informed, and meaningful research that truly benefits communities.<\/p>\n

Trusted Reviewers Project: Empowering Conversations for Thriving Lives<\/strong><\/p>\n

Through the Trusted Reviewers Project, we are listening to people\u2019s experiences in accessing day opportunities across Bournemouth, Christchurch & Poole. Our goal? To increase choice and control, helping individuals lead fulfilling lives.<\/p>\n

As a key partner in BCP Council\u2019s Fulfilled Lives Programme, we\u2019re ensuring those with learning disabilities, physical disabilities, mental health conditions, and older adults have access to diverse community care provisions that promote independence.<\/p>\n

The insights from this project are shaping future care and support services, reinforcing our commitment to person-centred care for all.<\/p>\n

Wellbeing Collaborative: Working Together for Better Health<\/strong><\/p>\n

Funded by BCP Council\u2019s Adult Social Care team, the Wellbeing Collaborative showcases the power of integrated working between hospitals, social care, and the voluntary sector.<\/p>\n

Together, we\u2019ve helped improve hospital discharge processes, connecting individuals with essential community support to prevent readmissions. One highlight? Our One-Off Wellbeing Grants, which remove discharge barriers and enable a person to return home promptly\u2014saving an average of 17 hospital days per person.<\/p>\n

This collaborative approach is transforming health and social care, demonstrating the incredible impact of working together.<\/p>\n

At CAN, we are passionate about addressing health inequalities through collaboration. Our partnerships with University Hospital Dorset, BCP Council, and community organisations ensure that people have access to the care and support they need to thrive.<\/p>\n

By bringing together expertise and resources, we\u2019re creating effective solutions and a more inclusive, equitable health system. We\u2019re excited to continue this journey\u2014proving that together, we can make a lasting impact!<\/p>\n
