Hypertension Optimisation (Dorset Population Health Management Approach)

This case study demonstrates how the Dorset Population Health Management (PHM) and Health Inequalities Team at NHS Dorset ICB have developed and applied a systematic approach to population health management (PHM) to rapidly improve hypertension optimisation.

The learning from this work enabling the development of a blueprint to support the ICBs Population Health ambitions, particularly in the context of Strategic Commissioning and Integrated Neighbourhood Teams.
