Dying Matters Week, (2-6 May) sees communities across the country coming together to talk about death, dying and bereavement.
In Dorset, local clinicians in collaboration with HCI (Health and Care Innovations) have developed a suite of four videos which provide an overview of things patients may wish to consider when thinking about making an advance care plan.
It’s never too early to talk about advance care planning. Having these conversations means that, as a community, we can work towards ensuring advance care plans are documented and that everyone can be #InAGoodPlace when they die – physically, emotionally, financially and crucially, with the right care and support.
